### Fixed
- Client disconnects during response writes (browser tab close, SSE reconnect races, mobile network switches, half-closed sockets) are now handled through a single `_CLIENT_DISCONNECT_ERRORS` set and a `_safe_write` helper instead of ad-hoc per-call-site `try/except`, so an expected disconnect no longer surfaces as a misleading server 500 in the logs. The error-response path is itself wrapped so a disconnect while sending a 500 is swallowed quietly rather than cascading. A bare `TimeoutError` from the Joplin notes integration's `urlopen` is now converted to a clean "not reachable" error at the route rather than escaping to the dispatch-level disconnect handler (#3210).
